Brief History

Records dating 1911 to 1930 were destroyed by fire in 1940

Restored files were under the custody of the Chief, Personnel Div. in 1941

Glasser Report stressed the importance of data compilation in medical research in 1968

UP Board of Regents set up a tripartite goal: patient case, training and research in 1971

National Kidney and Transplant Institute (NKTI)- Medical Records and Library Division (MRLD)

Brief History

Created in 1981 as the National Kidney Foundation of the Philippines and became operational in 1983

MRLD – maintains, manages, implements and develops medical records system

Provide sufficient medical and allied literature, and library services

San Lazaro Hospital- Medical Records Section

Brief History

Founded as dispensary in Intramuros by Fray Juan Clemente in1577

Became a hospital in 1578 serving patients with leprosy and other communicable and infectious diseases

Mandated by the Department of Health as the National Referral Center for dreaded diseases such as HIV/AIDS

Medical records have been maintained ever since by the hospital administrator but the records system was fully established in the early 1900

Holdings and Finding Aids

Medical patient records has been microfilmed since 1997

Morgue book (1919), register of admissions (1923), clinical charts and reports (1967 – annual, hospital statistics) – in paper format

Rare collections are clinical case and hospital statistics records

Retrieval by index cards, book of administration and disease index

Brief History

Non-stock, non-profit institution created in 1903 during the American occupation by Bishop Henry Brent, Head of the Missionary District of the Episcopal Church

Dr. Fabella Memorial Hospital- Medical Records Section

Brief History

Formerly known as the Maternity and Children’s Hospital and renamed in honor of the first Secretary of Health and Public Welfare, Dr. Jose Fabella, as per R.A. 5294, June 15, 1968

Medical Records Section started its operation simultaneously when the hospital started its operation in 1951

Prepares birth certificates for registration in the Civil Registrar’s Office

Dispose their records based on the approved Records Retention and Disposition Schedule approved by the Department of Health and the Records Management and Archives Office

Inpatient medical records – abnormal cases (since 1982) and normal cases ( since 1991); registers – admission ( since 1960 ), delivery (since 1959), operation ( since 1960), and birth ( since 1955); and indexes – master patient, and disease and operational, all in paper format

About the Author:
Emilia C. Calonge earned her Bachelor of Arts in Economics at Saint Louis University. She has sat as a member of the Executive Committee (Committee on Archives) of the National Commission for Culture and the Arts since 1992. She is the present Chief of the Records Department, House of Representatives.
