Pursuant to Commission on Audit (COA) Circular no. 2007-001, non-government organizations/people’s organizations, indigenous peoples’ groups, and individuals applying for the NCCA Grants Program are required to submit the following requirements prior to the evaluation of their proposal to determine their eligibility:

INDIVIDUAL PROPONENTS

• Comprehensive Curriculum Vitae with recent 2×2 colored photograph
• Record of Accomplishments/Projects Completed
• At least 2 Endorsement Letters from recognized leaders or practitioners in the field of culture and the arts from academic institutions or cultural agencies
• Tax Identification Number (TIN)
• Residence Certificate for the Current Year
• Bank Account Name of the individual
